What is Form 5 for in PF (Provident Fund)

Form 5 is a monthly report of the employees who joined the Provident Fund scheme during a particular month.

This form contains details of new employees who have become members of the Provident Fund scheme, their personal details such as name, address, date of joining, etc.

The employer is required to submit this form to the Provident Fund office on a monthly basis along with the monthly contribution.
